- Your nominated Star must be an employee /volunteer/service or department of a health or social care organisation providing publicly- fully funded services in Sunderland.
- We cannot accept nominations in a professional capacity from a social care or health service commissioner, employee or provider.
- The nominated Star must not be related to you.
- You cannot nominate an employee/service/department more than once for the same event and the event must have taken place in the last six months.
- We will not accept responsibility for loss or delay or in the incomplete nature of any nominations made using any method, either digitally or printed.
- Nominees will be notified and awarded a certificate if sufficient feedback has been given.
- Healthwatch Sunderland reserve the right not to honour Star nominations without giving justification or reason; the decision is final, no correspondence will be entered into.
- Healthwatch Sunderland will use all nominated Stars in their promotional activities including printed and digital media; ‘Stars’ may opt-out of this promotional activity.
- A Star nomination does not imply that Healthwatch Sunderland in any way endorse a service and this claim must not be made.
- The Healthwatch logo is trademarked and must not be used without written permission.
- Healthwatch Sunderland reserve the right to amend and/or withdraw the Nominate a Star programme at any time without prior notice.
- Entrants will be deemed to have accepted these rules and to agree to be bound by them.
- Healthwatch Sunderland reserve the right to add and amend these terms and conditions without prior warning or notice.
